Job Summary:

This job is responsible for planning, supervising and evaluating day-to-day care management activities at the assigned facility for the Franciscan Health System (FHS) to optimize quality of care and ensure effective utilization of resources and achievement of desired clinical and financial outcomes. An incumbent promotes collaborative practice and continuity of care and is also responsible for ensuring that all work activities are compliant with FHS policies and standards, and applicable regulatory/accreditation requirements. Work includes: 1) supervising professional RN and social work staff who are directly responsible for coordinating the care and service of selected patients; 2) triaging care management emergencies and assigning work coverage; 3) analyzing and coordinating care provided by staff, ensuring that adequate staff are available to meet patient needs; 4) monitoring documentation, quality, timelines and appropriateness of care; and 5) providing staff education for new policies and processes.

Work requires knowledge of the concepts, professional standards and methodologies applicable to the care management discipline, and of applicable regulatory requirements sufficient to ensure quality and compliant patient interactions and interventions care and exercise judgment/critical thinking in applying clinical guidelines and standards to determine the appropriate level of care, intensity of service, length of stay and place of service. Also requires expertise in supervising and evaluating the work of subordinate clinical staff. Major decisions (including policy changes) are subject to the review and approval of higher-level management.

Education/Work Experience Requirements

  • Bachelor’s degree in Nursing, Healthcare Administration or related field, graduation from an accredited school of nursing, and four years of progressively responsible related work experience that would demonstrate attainment of the requisite job knowledge/abilities. One year of experience in a lead/supervisory capacity preferred.
  • OR
  • Master’s degree in a behavioral health field (e.g. MSW, sociology, psychology, counseling, etc.) required and four years of directly related work experience that would demonstrate attainment of the requisite job knowledge/abilities. One year of experience in a lead/supervisory capacity preferred.
  • Alternative Requirement: A candidate who, although otherwise qualified, lacks a master’s degree may also be considered for the position if:
  • The applicant is actively pursuing attainment of a master’s degree in a behavioral health field, with a graduation date identified and continuing progress made towards completion of the degree requirements.


  • Active Washington State license as a Registered Nurse, OR
  • Agency Affiliated Counselor or one of the following:
  • LICSW - Licensed Independent Clinical Social Worker (Preferred)
  • LMHC - Licensed Mental Health Counselor
  • LMFT - Licensed Marriage and Family Therapist
  • LASW - Licensed Advanced Social Worker
